Registration for children living in the Cherokee, Northside, Westside and Youngs Grove Elementary School Zones will take place at the Polk School District Board of Education central office the week of March 18-22 from 8 a.m. to noon and 1 to 4 p.m. each day.
The central office is located at 612 South College Street in Cedartown.
Children in the Van Wert and Eastside Elementary School Zones will be registered at the Nathan Dean Center in Rockmart the week of April 8-12 from 8 a.m. to noon and 1 to 4 p.m. each day.
The Nathan Dean Center is located at 604 Goodyear Ave. in Rockmart. To register for kindergarten, a child must be fiveyears old on or before Sept. 1, 2013.
Parents are required to bring their child to the registration for a readiness and communication screening.
State law requires the following information to be presented to register a child for kindergarten: a certified copy of birth certificate, immunization certificates obtained from the health department and hearing, vision and dental certificates obtained from the health department.
If the child has an out of state birth certificate, it must be a certified copy with a state file number. If you have a non-certified copy of your child’s birth certificate, you will need to apply at the Vital Statistics Office in the state the child was born. This process takes six to 12 weeks to receive a new birth certificate.
Schools will also require a copy of the child’s social security card to be used as the child’s school ID number and a utility bill as proof of residency.
